Who it's for
Support That Doesn't End at Permanency
In Pennsylvania, families who have provided permanency through adoption — whether facilitated through the child welfare system, privately, or internationally — are eligible for valuable post-permanency services offered by the Statewide Adoption and Permanency Network (SWAN).
Formal kinship and permanent legal guardianship families who step forward to provide stability for Pennsylvania foster children can also access SWAN's post-permanency services — offered at no cost.

SWAN's post-permanency services empower families to identify their strengths, establish meaningful goals, and connect with community resources — ensuring continued support well beyond the completion of SWAN services.

Four interconnected services, shaped around what your family actually needs.
- 1
Assessment
Helps families identify strengths and needs, set goals, and create a family support plan. The assessment determines which units of services — case advocacy, respite, and support group — are most suitable. All family members, including birth children, are eligible.

Case Advocacy
Ensures families can access community resources that align with their unique needs. May include attending school meetings, fostering collaboration with other providers, and coordinating support services.

Respite
Supports eligible families by developing resources to alleviate the pressures of parenting children with special needs — establishing capable resources to provide ongoing care that remains available beyond SWAN funding.
- 4
Support Groups
Connect parents and children with other families and youth navigating similar challenges. Members form a community offering mentoring, education, socialization, and recreational activities.
How it works
One call gets you started
Families can access these family-driven services by reaching out to the SWAN Helpline. Helpline personnel gather essential information from the family and share it with the selected affiliate agency — so families never have to repeat their story when they meet their new worker. Once an affiliate is selected, a referral is made and the affiliate's worker will make contact within a week.

SWAN offers a post-permanency flyer that promotes Pennsylvania's free post-permanency services — accessible to adoptees (including those from private or international adoptions), permanent legal custodianship families, and formalized kinship families in Pennsylvania.
